I am not currently accepting insurance, but I can provide a superbill for reimbursement. A superbill is an itemized invoice healthcare providers offer to clients who pay out-of-pocket for services. It contains essential details to help clients seek reimbursement from their insurance companies. Here's what you should know:

What is a Superbill?

  • A document given to you by your therapist summarizing the services provided.

  • Used for insurance reimbursement if your therapist is out-of-network.

Key Information Included:

  1. Provider Details: Name, credentials, license number, and NPI (National Provider Identifier).

  2. Client Details: Name, date of birth, and possibly an insurance ID.

  3. Service Information: Date of service, type of service (e.g., individual therapy), and CPT (Current Procedural Terminology) codes.

  4. Diagnosis Code(s): Required by insurers to process claims.

  5. Payment Details: The amount paid and the provider's signature.

How to Use a Superbill:

  1. Please reach out to your insurance company to confirm out-of-network coverage for therapy.

  2. Submit the superbill along with any required claim forms to your insurance provider.

  3. Could you monitor the reimbursement process for updates or additional requests?
